The Domiciliary Care Coordinator is responsible for organising and overseeing care services delivered to individuals in their own homes. The role ensures high-quality, person-centred care while coordinating care staff, managing schedules, and maintaining compliance with regulatory standards.
• Develop, implement, and review individual care plans tailored to service users’ needs.
• Liaise with service users, families, healthcare professionals, and social services. • Conduct initial assessments and risk assessments. • Monitor and update care packages as needs change.• Create and maintain rotas to ensure safe and efficient care delivery.
• Match care workers to service users based on skills and compatibility. • Manage staff availability, sickness, holidays, and emergency cover. • Support recruitment, induction, and ongoing training coordination.• Ensure services meet regulatory requirements (e.g., CQC standards in England).
• Conduct spot checks, supervisions, and performance reviews. • Maintain accurate records and documentation. • Investigate complaints, incidents, and safeguarding concerns in line with policy.• Act as the main point of contact for service users and families.
• Build positive working relationships with local authorities and healthcare providers. • Respond promptly to queries and concerns.• Maintain care management systems and databases.
• Prepare reports for management and regulatory bodies. • Monitor hours delivered and assist with payroll information where required.
