Registered Nurse (Domiciliary Care)
Reports to: Care Manager
To provide high-quality, person-centred nursing care to patients in their own homes, promoting independence, dignity, and well-being while ensuring safe and effective clinical practice in line with professional standards.
Key Responsibilities
• Deliver comprehensive nursing care in patients’ homes.
• Assess, plan, implement, and evaluate individualized care plans. • Administer medications safely, including injections and controlled drugs where appropriate. • Perform wound care, catheter care, PEG feeding, and other clinical procedures. • Monitor vital signs and identify early signs of deterioration. • Manage long-term conditions such as diabetes, COPD, heart failure, and dementia. • Provide end-of-life and palliative care where required.• Conduct holistic assessments considering physical, emotional, and social needs.
• Develop and review care plans in collaboration with patients, families, and multidisciplinary teams. • Maintain accurate, timely, and confidential clinical documentation.• Safeguard vulnerable adults in line with safeguarding policies.
• Adhere to infection prevention and control standards. • Ensure compliance with NMC Code of Conduct and organizational policies. • Report incidents, concerns, and changes in patient condition promptly.• Liaise effectively with GPs, community services, social workers, and allied health professionals.
• Provide health education and support to patients and families. • Supervise and support healthcare assistants where appropriate.• Maintain active NMC registration.
• Participate in clinical supervision, training, and continuous professional development (CPD). • Engage in reflective practice and quality improvement initiatives.
